| Plea of Insanity as a Defense in Criminal Cases : An Update Sayeed Akhtar, Tushar Jagawat January-March 1994, 36(1):25-29 PMID:21743662Mc'Naughten Rule is die commonest formulation for plea of insanity in most countries. Although assessment of the mental state at the time of offence is difficult, few instruments have been devised for this purpose. In the USA, efforts are being made to abolish the insanity defense and in the United Kingdom, amendments in the provisions of insanity defense have been proposed. The authors have stressed the need for some changes in the relevant sections of Indian Penal Code to increase the credibility of Psychiatric testimony in such cases. |

Various etiological, developmental and personality factors in the genesis of anxiety have been discussed. The patient was treated with anxiolytics, antidepressants and dual sex therapy and was followed up for five years. |

| Risk Factors for Tardive Dyskinesia Sunil Datta, TP Subhalakshmi, L Jeyaseelan, K Kuruvilla January-March 1994, 36(1):22-24 PMID:21743661Three hundred and fifty three patients who had been on antipsychotics for three months or more were assessed using Simpson Tardive Dyskinesia Rating Scale. 40.2% of these patients, who merited a diagnosis of probable Tardive Dyskinesia (TD) by the Schooler and Kane criteria, were reassessed three months later and 25.5% of the total sample were found to have persistent TD. Age, total antipsychotic dosage and duration of antipsychotic exposure were found to be positively correlated with persistent TD. |

| The Perception and Experience of Health Personnel about the Integration of Mental Health in General Health Services Arun N Naik, Mohan K Isaac, R Parthasarathy, BV Kumar January-March 1994, 36(1):18-21 PMID:21743660The National Mental Health Program of India focuses on the integration of mental health in general health services. Using a structured questionnaire, 100 health personnel (40 Medical Officers and 60 Health Assistants) in the District Mental Health Program were interviewed regarding their perception and knowledge about the integration of mental health in general health services. Most personnel were found to be satisfied with their new role in carrying out mental health services and only a few felt that it was an extra burden. Suggestions were made by them about the free supply of drugs, short and long term training and active supervision and support by the higher authorities. |

| Multifocal Leucoencephalopathy Presenting as Antidepressant Induced Mania Saji S Damodaran, Vinod Kumar Sinha, Arunava Das, Sayeed Akhtar January-March 1994, 36(1):30-32 PMID:21743663A male patient presented with tricyclic induced hypomania after treatment for chronic depression. On follow up the patient deteriorated towards dementia with multifocal leucoencephalopathy. The possible role of brain lesions in the causation of affective syndromes and the neurobiological hypothesis of bipolar syndromes are discussed. The authors propose that chronic depression should be thoroughly investigated for a probable Axis III diagnosis. |
