Psychiatric Aspects of Clinical Practice in General Hospitals : A Survey of Non-Psychiatric Clinicians

The present work was carried out to study the awareness of non-psychiatric clinicians working in a teaching general hospital about the frequency of psychiatric morbidity in their clinical practice, their utilization of psychiatric consultation services, and opinion about utility of liaison psychiatry in general hospitals. A substantial proportion of doctors underestimated the psychiatric morbidity especially about unexplained physical symptoms and specific depressive symptoms in their patients. Psychiatric consultation services were not sufficiently utilised by a large number of clinicians. Most ofthemfelt the need to improve upon undergraduate medical education in psychiatry in India as well as a desire to have consultation - liaison psychiatric units in India.

Sexual Abuse in Childhood : A Report of Four Cases

Sexual abuse of children and adolescents is prevalent across all cultures. Sexual abuse often begins in infancy or toddler stage for many children. A Child's developmental level influences the detection and disclosure of sexual abuse. Pre-school children very rarely make verbal disclosures, but may present with behavioural and physical symptoms. Older children may make a conscious decision to reveal the abuse, but often feel that they risk their safety in making a disclosure. This paper reports four cases of sexual abuse detected in a clinic during a period of 12 months. The age group of these children ranged between 4-12 years.

Suicidal Ideators in the Psychiatric Facility of a General Hospital - A Psychodemographic Profile

As a part of the basis for planning of prevention of suicide and suicide attempt, 154 suicidal ideators registered between 1988 and 1991, were recruited for an analysis of their psychiatric morbidity pattern. 59.74% had depression. The representation of substance abuse disorder and psychoses were 9.74% each 7.14% had neurotic disorders. 9.09% had bipolar affective disorder and 0.65% had normal mental status. 40% of the sample were housewives. Majority of them were between the age group of 16 and45years, having an educational status below 10th Standard The four year follow up findings showed that 8 of the sample completed suicide, of which four were suffering from psychosis. Repeaters suffered from disociation disorder. 15 of the depressives had resistant depression with nonremittent suicidal ideas. Psychotic patients and patients with somatic complaints were not free communicators of their suicidal idea. Implication of the results in the cunicalmanagement and further research on these patients are discussed.

50 divorce seeking couplet, when compared with 30 well adjusted couples, showed that sex-related factors and sexual dysfunctions were related to divorce seeking behaviour. A significantly high number of couples in the study group, reported a bad honeymoon, unsatisfactory coital experience, lack of cooperation from the spouse and variant sexual habits. The findings emphasis the importance of imparting adequate knowledge about sex and management of sexual dysfunctions, as part of marital therapy.

My dear colleagues, ladies and gentlemen, I consider it a great privilege and pleasure to address you this morning as the next President of Indian Psychiatric Society. I am most grateful to the fellowship of the society for bestowing their trust and confidence in me and having elected me to the highest position in the profession at the national level, which had been a cherished dream for me. I feel humble when I think of the great stalwarts who have been the past Presidents of this great society. Amongst this galaxy of luminaries are my mentors, senior colleagues and friends who had contributed in a big way to the development of psychiatry and moulded the mental health services in our country. I shall endeavour to hold the great traditions led by them for the furtherance of the aims and objectives of our society. I have been participating in the affairs of the society for more than twenty years now, and was actively involved in different capacities as a fellow, member of the executive council, the honorary treasurer, as Vice President and now as the President. With your good wishes and kind co-operation I shall try, to the best of my capacity, to carry out the onerous duties and responsibilities that you have entrusted to me. Each of my honorable predecessors had delivered their presidential address on a specific theme; I too will like to do the same. The theme of my today's address is, "SPIRITUALITY, SCIENCE & MENTAL HEALTH".

Once Daily Lithium in the Prophylaxis of Mood Disorders

This retrospective chart review attempts to compare the utility, safety and efficacy of once daily (OD) versus divided dose (DD) lithium therapy in the prophylaxis of mood disorders. Sixty-six patients who met DSM-II1R criteria for mood disorders were grouped into those on OD Lithium (n=31) and those on DD lithium (n=35). The groups were matched on sociodemographic and clinical variables. The total daily dose was similar in the two groups, but the OD group tended to have higher serum lithium levels while the DD group tended to have a greater number of affective episodes during the period of follow up. The implications are discussed.

The Neuroleptic Malignant Syndrome : Report of Three Cases

Three patients, one male and two females who developed neuroleptic malignant syndrome following exposure to depot and oral neuroleptic are presented. The patients satisfied Levenson's criteria for diagnosis. Bromocriptine and Electro Convulsive Therapy were found effective whereas trihexyphenidyl proved unsatisfactory in the treatment. All the patients recovered completely without complications.

An analysis of sales of psychotropic drugs in 1 year in Warangal district showed maximum sales of Fluoxetine. Total dopamine receptor antagonists sales, by the amount of money spent, were less than, that of Flutoxetine.
